If the number and position of the remaining teeth does not permit the making of a fixed prosthesis (bridge), the patient receives a removable prosthesis. These are dentures the patient can remove from their mouth without the help of a dentist. If the number and position of our patient’s remaining teeth or installed implants do not permit the making of a fixed prosthesis (bridge), then plated tooth replacements can be used to restore the chewing, speaking and aesthetic function. In these cases the load of chewing is borne not only by the teeth but also by the mucous membrane and bone foundation. Plated prosthesis is made of a thin metal “base-plate” which is put as far as possible in the least disturbing place. The technician casts the prosthesis base-plate and clips from cast metal, and the artificial plastic gum and teeth are connected to this. This type of prosthesis is less harmful for the remaining teeth, and by applying crowns on the clip-holding crowns it is an acceptable replacement also in the long run. Its disadvantage is that the cast clips cause an aesthetic problem. This type of prosthesis is fixed to the teeth by clips therefore it is comparatively rarely used; however, combined with a bridge, we can offer our patients well-fixed tooth prosthesis. We rigidly integrate the remaining teeth with a bridge, and the removable metal-frame superstructure is connected to the bridge by hidden anchorage. It is generally applied if there are enough remaining teeth in the territory of the front teeth while teeth are missing at the side areas. The removable prosthesis is connected (as a snap-fastener) to the bridge through a little fixing part situated inside the prosthesis and is therefore invisible from the outside, which can be easily put into its place in a single step. The artificial teeth and the artificial gum part of the prosthesis are made in a way that they harmonize with the shape of the remaining teeth and the teeth of the bridge prosthesis. The hidden anchor provides an excellent aesthetic effect. It is recommended for patients for whom no implant can be installed in the area of the side teeth, or who do not want to undertake the implant operation and its costs. Telescope anchored dentures If only 1-3 teeth are left, the telescope anchored denture is the best solution. The remaining teeth are prepared by the dentist and on the basis of the provided impression the technician makes the primer and secondary telescope crowns that perfectly fit each other. The telescope crowns serve to increase the efficiency of the fixing; in dentistry they are most often used to increase the stability of removable prosthesis. The telescope crowns are built of two parts: the primer crown is made for the remaining polished tooth and is cemented thereon, while the secondary part is situated in the removable denture and is a precise negative copy of the primer part. The primer and secondary parts fit into each other as a key into the lock; the efficiency of the fixing is ensured by the frictional crown walls. The telescope denture ensures a favourable effect both in function and aesthetic outlook.
Full plated removable prosthesis – denture Full plated removable denture is a removable prosthesis that is used in case of a total lack of teeth. It is one, at present the most often used method for the restoration of a total lack of teeth caused by the loss of teeth. The full removable plated prosthesis is made into a toothless mouth. Considering its fixedness, the anatomical conditions of the upper jaw-bone and the suction effect of the palate provide favourable conditions for the holding of the denture in place. However, in case of the lower set of teeth, the making of a stabile denture can run into difficulties due to frequently unfavourable anatomical conditions. The base material of the full plated denture is a special plastic. First it may be uncomfortable to wear, but this feeling later ceases. Unfortunately, due to the continuous decay, atrophy of the toothless jaw-bone, the padding of the full plated denture after 1-2 years and its complete change after 5-7 years may become necessary. |
